The Old World is commonly thought of as a place of monarchies, and the New a place of republics. While this is generally true, there are exceptions in both cases. One of these is the Mexican Empire.

You are asking what if the Aztec Empire ended up being a republic? Never would of happened on their own and even if they got the notion from Europe it would never happen either.

Even when Mexico gained independence in 1820s and adopted the guise of republic it could be argued it has never been anything but a failed state of countless revolutions and corruption.

It needs to be an empire to live. The conservative aristocrats whose money the government used to industrialize it couldn't accept a republic, and the poor who would accept one didnt have the resources to prop it up until Augustin II's reign
 
Technically the Aztec system was a republic until they defeated the conquistadors, although the "Speaker" always came from the same family. Once they converted to Christianity they also adopted a European style hereditary monarchy.

Obviously the POD should be specified. The Wilsonian intervention is the most likely one, but that is post 1900 and a bit too late. There were five serious revolutions or revots against the royal dynasty that could serve as the POD, plus you could have any of the Spanish or French invasion succeeding and then reversed by a nationalist revolt that does not restore the dynasty.
