Rhaenyra decisively wins the Dance of the Dragons.
Rhaenyra was practically insane, and would be a “Mad Queen” in opposition to Mad King Aerys II, but by far not the worst scenario that could occur. Ignoring Westeros, and looking at the whole world view. A White Walker victory would result in complete destruction of the previous world. We think it would only result in Westeros’ collapse, but the White Walkers would come to Essos as well. There is a far eastern passage that (theoretically) connects to the Lands of Always Winter, but regardless of that, there are creatures similar to the White Walkers on that side of the world. Yi Ti (ASOIAF’s China analogue) had a legend similar to Azor Ahai, which confirms they have such superstitions from the past. If the White Walkers win in Westeros, there would likely be an invasion of Essos as well. Either across the Narrow Sea (assuming they can traverse it) or from the Eastern Passage I mentioned. If there is no Azor Ahai either (likely if there is no Stannis, Daenerys, or Jon), the White Walkers basically can’t be stopped. It happened in the show that they stopped them, but they had Daenerys, Jon, and Melisandre to help. Essos has no dragons anymore (without Daenerys, those unborn eggs are useless), so they’d be powerless to stop the invasion. Sothoryos and the Summer Islands may survive, but Sothoryos is uninhabited essentially, and the Summer Islands depend on Essosi trade.

There's worst scenarios than that as well
I think the worst thing that could happen in HP(granted, that could be caused by the Death Eaters winning as well) is a Dementor uprising taking over Britain and possibly winning against the rest of the magical world
It's hard, of course, as most wizards are at least aware of the Patronus spell, but if they were weakened enough or if the dementors reached numbers that far exceed what the wizards can fight back against then the whole world could be infestated with soul-eating joy-sucking indestructible eldricht abominations that cant be even PERCEIVED by normal humans
It'd be a world ruled by Rowling's representation of depression that would either drive humanity to extinction by eating the soul of everything that has one or they would use their limited intelligence to take a page out of our book and breed us like we do with cattle to always have a meal while we live in a eternal state of misery
And considering they're born from emptiness itself and are immortal...they could outlast the death of the universe
 
FRIENDS: Ross snapp when finding out his boss ate his sandwitch. Stalks the boss after work and breaks into his home and kill him. Then go to Monica where the gang is and take out a flame thrower and fry them(Ross is not strong enough to take on any of the others in a fist fight, maybe Chandler and maybe Rachel). After setting fire to the house he runs away and become a serial killer. Before the authorities know it is him he gets on a plane to Afghanistan and meet Bin Ladin and Ross become the master mind of Al-qaida and making them think bigger than the WTC. Instead he gets Bin Ladin to get plutonium and scientists. Then Ross return home to celebrate 4th of July in 2004 or so with big fireworks in New York

Aldenta: Sheva is destroyed and the German led task force never return to Earth allowing the Posleen to overcome the last human defenders.

West Wing: Zoey is kidnapped and killed by extremists from a middle eastern country. Israel believing that they will become the heroes in USA nuke said country. Iran etc retaliate. Israel nuke some more. Russia is dragged in forcing USA to launch their nukes.
 
Star Wars: Erza joins Emperor Palpatine and gives him access to the world between worlds. Palpatine would gain control over time and space, a time traveling Palpatine means game over for the forces of good.

Transformers GI cartoon: Optimus Prime does not return to cure the Hate Plague. The every person in the universe seeking to destroy forever.

Dc comics: The Anti monitor wins, no more DC Universe.

Dont think he would even need to time travel
Just being there means he can use his dark side aura to corrupt every world/timeline and manifest his influence in every single single point in space and time, he'd be omnipresent and being as strong as he is with the Force that would likely make him omnipotent*
We wouldnt be getting Emperor Palpatine, we would be getting Demiurge Sidious and instead of the Empire we would have all existence as his personal hell

*In the sense of being all powerful, not "I can do anything even if it doesnt make logical sense"

Many of the obvious "Worst Possible Timelines" follow the same basic script: "Rocks Fall, Everybody Dies". Two examples:

Star Trek Novels: In either The Entropy Effect or The Wounded Sky, the good guys don't manage to save the day and the universe effectively ceases to exist, either through too much entropy or a complete lack of entropy (respectively).

Discworld: In The Last Hero, the pointless albatross sent from the Agatean Empire fails to reach Lord Vetinari in time, and the Silver Horde destroy Cori Celesti with explosives, inadvertently wiping out all life on the Disc.
 
The X-Files, the alien invasion in December 2012 actually happens instead of being ret-conned. I like to think that The After (made by the same creator) is set in that universe and depicts that turn of events (since it was filmed around the same time, late 2012).

Avatar: The Last Airbender: Aang fails and the Fire Nation succeeds. Alternatively, Zaheer succeeds in ending the Avatar cycle as well as, eventually, order as a whole and the world descends into chaos.

Star Wars: The Rebels succeed, create another worthless Republic, which descends into another civil war and a fascist dictatorship emerges to gain control over a post-imperial galaxy. ...Hey, wait a minute!
 
For Deltarune, Noelle is forced to go through the Snowgrave route, Kris finds themself totally controlled by the player, and Noelle has a breakdown upon realizing what she did in the Dark World, causing the player to use Kris to push her into killing Lightners as well.

I thought Evangelion already had a bad ending(s), non-sensical as they already are.

Attack on Titan:
Eren isn’t stopped in the final arc and destroys the entire planet instead of just 80% of it.

Nightmare Before Christmas:
Jack dies when he’s shot out of the air. The houses he visited are terrified and people hurt or killed by the toys he left. Oogie kills Sally and Santa. Christmas no longer has a Santa and Oogie has more of a free hand to meddle not only with Halloweentown, but also the other holidays now that his goons know where the doors are.

Lord of the Rings:
The Battle for Middle Earth games pretty well summarized the worst case scenario there. Death, suffering, and slavery grip the entirety of the realm. The only being capable of stopping Sauron being Tom Bombadil if he bothered to care.

Tenacious D and the Pick of Destiny:
The D doesn’t play the best song in the world and Jack doesn’t block the lightning bolt from Satan. KG is sent to Hell to be a sex-slave, Jack dies, and the world falls to Satan.

James Bond:
Basically every movie except for the Craig films would result in at least one city going up in flames if not a full-blown WWIII. The Craig films kind of already have the shadow cabal largely in control of world events so they just solidify that and there’s more suffering in the world for the 99%.

Team America World Police:
Gary remains a drunk instead of trying to set things right. Kim Jong-Il isn’t stopped in his plan to launch an attack on the scale of 9/11 times 2,356; a plan so evil that nobody even knows how bad it is, except every country would be a third-world country.

The Katsuragi expedition fails to subdue Adam in 2000 and she generates a worldwide Anti-At field, thus wiping out all Lillith-based life

Regarding Eva
If I'm not mistaken the OG angels such as Adam, Lilith and so on were sent to bring life to other solar systems by a long gone ancient civilisation in a effort to keep sentient life alive
If they failed or perished before they could atempt that you prevent the existence of all life on Earth, lilin AND angel, as well as that of any other planet fertilized by them
That could quite possible leave the entire universe devoid of life as we dont know if any world in the Evaverse aside from theirs had the conditions for living creatures to evolve on their own
 
I don't have to invent one for Avatar The Last Airbender, because there is already a fanfiction for this.

To sum it up:
- Zuko is able to capture the Gaang during season 1 and brings them to the Fire Nation, ignoring Iroh's suggestion that this a bad idea

- Sokka and Appa are executed, Aang is blinded and has his hands cut off to make sure he will stay alive and never use his powers again and Katara is used for training Fire soldiers in the arena where she quickly develops a reputation fro the number of people she kills. Meanwhile it is implied Toph had a fate like Katara but died earlier

- Zuko realises too late his father doesn't care about him and the court believes Iroh captured the Gaang but simply let Zuko take the merit. Out of guilt he becomes an alcholic and ends up living in a fantasy world where he sided with the Gaang and ended the war with them

- Azula is crazy as usual and besides enjoying the fact Aang went insane in his cell, she has also been put in charge of "dealing" with the Water Tribe

Granted dark fic are rather popular with the ATLA fandom, including a couple where Aang goes insane and destroy the world

Terra Prime infiltrate the upper echelons of Section 31 and maintain the grip on power through the next hundreds of years. In the 2250s, they plant explosives in the Qo'noS crust, causing the volcanic system to awake and create a distraction for the Klingon Empire to stop the war with the Federation. (Burnham's mission is just a cover-up available for wider audiences). Later, they dissolve most of their ranks to strengthen the secrecy of the agency to blow up Praxis, the moon of Qo'noS and cripple the Klingon state. With greater inter-species integration of UFP, lower echelons composed of multi-species agents are kept in dark about the origins of the agency and manipulated. The Dominion War starts with the discovery of S31 interference in the Changeling pandemic - while it ends with an armistice, during the next dozens of years Changelings would mostly die. In 2387, Section 31 cause the destruction of Romulus by delaying Spock flight. Admiral Kathryn Janeway's flight to the past carry an S31-engineered virus to critically weaken the Borg Collective during the tensest period of galactic history.

However, there is a similar organization in the universe - Zhat Vash that learnt about the S31 human supremacist plans when S31 planted their operative as the Chairman of Tal Shiar, a lesser intelligence agency. Both S31 and Zhat Vash allow the failed attack on the Dominion by Tal Shiar - Obsidian Order alliance to eliminate possible opponents during the final fight. The Star Trek" Picard would see the final war between two most powerful secret organizations in the Galaxy - Zhat Vash that wishes to destroy the synthetic lifeforms and secure independence and Section 31 that wants to eliminate all threats to humankind and assimilate species regarded as worthy to be the second class citizens in the New Galactic Order. Little do they know that the synths carry their own plans to achieve unlimited freedom from their former, current and possible future oppressors.
 
Final Fantasy 7: Aside form the rather basic possibility of Sephiroth winning at the north crater, meteor impacting and Sephiroth becoming a god by absorbing the lifestream, I could also imagine a timeline where he does not get sent to Nibelheim, never learns the truth and nobody ever gets in a position to successfully oppose Shinra which then rules over the planet with an iron fist while slowly killing it.

Warhammer Fantasy: Chaos wins and gets to destroy reality! Wait a moment...

Puella Magi Madoka Magica: this is more difficult because Homuras time resetting powers make the ending very much dependant on her and her willingness to try again. Maybe she tries so often to get a better timeline that she goes mad or finaly gives up. Madoka, being saddled with all the Karma from Homuras failed attempts makes a pact with Kyubei with a mostly meaningless wish, inevitably becomes a witch at some point and basically ends up destroying the world/universe.

Space Battleship Yamato: Rather easy, the Gamilas destroy the Yamato before it reaches Iscandar and humanity goes extinct.

This could be a good spin-off premise
Like Madoka wishing Homura was happy and then becoming the universe-destroying witch, causing the now revived Homura to be horrified and have the will to try again while the Witch Madoka keeps sending some kind of avatar in every timeline that Homura creates to ensure everything goes wrong and she's not butterflied away while feeding on all the karma Homura creates in her every atempt. Tldr: Madoka Magica Terminator-style.
"A Meaningless Wish" could be a good title as well.

Anyways sorry for my fanfic-ey rant, though I think this could count as a pretty bad ending.
