Not sure of this counts, but there was an incident near Goldsboro, North Carolina, in which a B-52 crashed whilst carrying two 3-4 megaton nuclear warheads. If one or both had gone off, America would've nuked itself.
Some field exercise in Midwest is interpreted by right wing extremists in Texas as a UN invasion of the US and hundreds of Texas form militia and attack them along Texas birder. Hundreds of people are killed or injured before things are calmed down. Texas governor escalated things even further during the heat of moment and call up the state guards to defend Texas causing even more confusion and shooting incidents

IIRC isn't it more of a kind of thing where, since you most likely aren't going to actually face an invasion from north or south on US soil, it's more of wargames to be used in individual instances rather than a serious invasion threat? Unfortunately there is a segment of the US population who believe that US will be invaded by the UN and that the UN troops are already in the US housed in secret bases or just waiting over the border. While in the past we always believed that it was a few "crazies" who thought that then last year or year before the Texas governor brought this up and talked about calling up national guard in Texas to guard against the invasion. So unfortunately the numbers are higher than just fringe group and some people of power also believe it.

In 2002 a North Carolina deputy killed one US soldier and wounded another after mistaking them for criminals during a live field exercise. Several similar incidents have occured throughout the years. What is the worst possible outcome of such incidents if taken to an even greater extreme?

"Canada" had/has "socialist medicine" and a "liberal" government so of COURSE they are closet Commies ready to support the Ruskies and the UN in taking over the US! And by the late 80s EVERYONE was expecting the attack to come from South of the Border so again it would come from NORTH of the border!
As noted things could very easily go downhill quickly when you mix a large scale military exercise, militias and bad government decisions. The military blows off public 'concerns' over Jade Helm, (like they almost did OTL) and the Milita movement puts 'observers' on the ground and builds up manpower in the exercise area. Close proximity and escelating incidents, (like stealing food and ammo from the military which isn't unlikely) leads to words and then action and suddenly you have a state governor declaring an "emergency" and putting the State Guard in support of someone like the Texas State Milita against the Federal forces and it escelates from there.
When the State government is 'siezing' Federal factilites and equipment and arresting or worse Federal Troops and employees, and the "media" is telling everyone those same people are shooting your neighbors for refusing to surrender their guns and/or raping and murdering anyone they catch and "someone" has been telling you for years this is what is coming and you need to fight it when it does...
There's a good reason why these big exercises activily include the State Guard in the activities and why the military now takes "citizens concerns" seriously and does a lot of preparing and networking to avoid such possibilities.
Imagine if today's Milita's woke up to a Trigonist "invasion" like what happened in Texas in 1952?
