The 1800 election saw a nasty campaign against Jefferson and Adams. As the 12th amendment wasn't established yet, there was a unique situation where the nominal running mate could potentially get the presidency instead. This almost happened with Aaron Burr. What if it did happen?
There are a few things I'd think would take place. First, Burr would have to face accusations of being an illegitimate candidate, and the relationship between him and Jefferson would be far more bitter(and Jefferson especially as he won the proverbial battle with Adams but lost the war). The 12th amendment would go up anyway in response to this, I'd think. It would make it extremely difficult for Aaron Burr to get re-elected, either leading to Jefferson being the Democratic-Election choice or someone else chosen for that position(George Clinton, maybe?). Ironically Hamilton would never be killed in that duel(even he wouldn't dare duel the president).
