France needs another excuse to try and annex Lorraine.

No War of the Austrian Succession, which means weaker Prussia without Silesia.

Charles Albert never becomes Holy Roman Emperor.

After that, so many potential butterflies.

Britain and France will still probably have another war in regards to India and North America.
 
What if "Maria Theresa" was a boy? How might that have changed history?
First with either Maria Theresa born as boy or her older brother surviving you are butterflying the war of Austrian Succession and the Prammatica Sanction plus all the concession who Karl VI was forced to made for getting support and recognition for the succession of Maria Theresa
In second place you are preserving the true Habsburg dynasty in Austria and at the same time butterflying both the French acquisition of Lorraine and the Prussian annexation of Silesia while keeping an uninterrupted succession of Habsburg Emperors in the HRE (no Charles Albert of Bavaria as Karl VII here and so no concession to the nobility and Imperial diet, so the HR Emperor will keep, among the other things, the right to give full dynastic rights to a not-Hochadel wedding).

male Maria Theresa (likely Karl, Leopold, Ferdinand, Maximilian, Johann or Joseph) or Leopold Johann will likely marry Mariana Victoria of Spain after she was rejected by Louis XV (no need to marry in Portugal when you can be Empress, while one of her younger sisters will marry in Portugal) likely in a double match with Maria Anna (or Maria Theresa if we go with the survival of OTL brother) and don Carlos


In the end: the Holy Roman Emperor will not lose a part of his powers, Lorraine will not relinquish their Duchy to France, Prussia will not annex Silesia, likely better Austrian-Spanish and worse Franco-Spanish relationship than OTL, no Diplomatic Revolution and no Family Compact (as either England will not have any interest in exchange Austria with Prussia at main ally or Austria and Spain will become each other main ally if they can reach an agreement about Italy)
