I don't see much difference. After all, all that would change is that she would rule together with Phillip the fair rather than being "regencied", and she would still likely heed her father's advice regarding the ruling of the kingdoms. Charles would still be the heir.

Her personal situation would have been less cruel and tragic, but i don't see any consequence that would trascend that.

Maybe the Comunidades rebellion could be butterflied away, if Charles was brought up in Spain rather than in Flanders?

Well a lot would change. If Juana ruled directly Charles wouldn't have access to Spanish resources for most of his reign. In that case Charles would remain in Flanders and the Empire, with spuratic visits to Spain and possibly Italy. I wonder if it would be better or worse if Charles remained focused on the Empire, ruled it directly.
 
Then he'd run into the same problem as OTL, with the rebellions, but at a later time.

You don't think a "sane" Juana would make it more likely for Charles to grow up with her? I don't really know how this transpired OTL.
 
Charles could be raised in Spain, so the electors decide to choose a non-Spanish candidate, perhaps Ferdinand or even Francis I!!
