WI: Francois Duvalier dies in 1959?

This is something interesting I came across browsing Wikipedia:

On May 24, 1959, [Papa Doc] Duvalier suffered a massive heart attack, possibly as a result of an insulin overdose; he had been a diabetic since early adulthood and also suffered from heart disease and associated circulatory problems. During this heart attack he was unconscious for nine hours; many associates believed that he suffered neurological damage during these events that affected his mental health and made him paranoid.[15]

So...a plausible opportunity to use a heart attack to change the course of history, probably for the better. Suppose Duvalier dies from the heart attack rather than surviving by the skin of his teeth. Haiti is without its President, and more importantly without the family that would rule over it for more than a quarter-century afterwards. What happens?
 
At 19 years old, Jean Claude Duvalier becomes an overweight playboy instead of President of Haiti.

With Papa Doc's death in 1959, there won't be any Tonton Macoutes.

Otherwise, there will be endless military coups and sham elections post-Duvalier.
