In 363 Emperor Julian was mortally wounded in a clash with the Persian army near Ctesiphon and died the next day.
WI Julian is never wounded or his wound is superficial and he is healed?
What then? How is a surviving Julian affects the battle between paganism and christianity?

Judging by his efforts as of his death, Julian is unlikely to make paganism succeed - whether he gets overthrown by a Christian is an interesting question.

In OTL Julian didnt had an heir... It would be interesting also if fathered a son...

This is true. Who would he marry, and why?

I say why because it doesn't seem to have been on his list of priorities, though he might have done it if he'd reigned longer.

Though highly improbable, the only way I could see paganism surviving would be the way Judaism did: lots of discrimination and pogroms.

He was already married since 355 AD to his cousin Helena daughter of Constantine I and Fausta...

During 360 an uneasy peace simmered between the two emperors. Julian spent the 360 campaigning season continuing his efforts to restore order along the Rhine, while Constantius continued operations against the Persians.[[46]] Julian wintered in Vienne, and celebrated his Quinquennalia. It was at this time that his wife Helena died, and he sent her remains to Rome for a proper burial at his family villa on the Via Nomentana where the body of her sister was entombed.[[47]]

I have forgotten Helena's death...

With Helena dead the problem is that Julian quit sex entirely and lived a celibate life... So if he survives and lives past 363 the problem (beside the struggle between christianity and paganism) would be who would succeed him...
