Joanna's closer to Charles V's age than Mary was IOTL, so maybe instead of Mary being the one betrothed to him, it's Joanna?

Prince Henry might marry Renee of France. Alternatively, if Henry insists on a Habsburg alliance, it could also be Catherine Habsburg.

Both Henry VII's daughter Mary Tudor and Henry VIII's daughter Mary Tudor were considered as a potential spouse for Charles V.
Mary the elder was born in 1496, Mary the younger was born in 1516, whereas Charles V was born in 1500.
IOTL Charles V was in need of an heir of an heir, but he married in 1526.
So even TTL Joanna would be pushing it, still a few years older than her sister Mary, but Charles V really couldn't afford to wait very much longer. Their aunt Mary would have been the safest bet, but Joanna could be a serious consideration, whereas Mary the younger never was a serious option.
 
Joanna wuold be good as age...
Sure Isabella of Péortugal was 22 at the time of her wedding with Charles but at that time a princess can be legally married (and most important consumate the marriage) already at 14 years so...
 
Assuming Catherine of Aragon was able to produce three surviving sons, her position would be completely unassailable and its likely she would instill in all her children (as she did indeed with her daughter Mary) a sense of their Spanish heritage, which would likely lead to a lot of first cousin marriages with the children of Catherine's siblings.

I also suspect one of the daughters would marry her first cousin James V of Scotland.
