According to tradition, Etruscan Kings, Tarquinius Priscus, Servius Tullius and Tarquinius Superbus, governed Rome for over one hundred years. It has been suggested that Rome actually became a city with permanent defences over that period and families with Etruscan names remained important in the Early Republic. So what would change if Etruscan had become the dominant language of Rome? Perhaps it would have been harder for Rome to lead the Latin league. Possibly the Etruscan cities would have accepted Roman leadership more readily. However, assuming that Rome expands more or less as OTL, would regions such as Spain, France and Romania have adopted a language which was not of the Indo-European family? What would Claudius have written in his spare time?
