Even though George held his cousin in contempt, and the continued rise of Germany was terrifying to British leadership
It is true, that if George was intending on siding with one of his cousins due to familial ties, it would most likely have been with Nicky.... George and Nicholas actually liked each other, while both sort of thought Willy was an obnoxious boor :p

First of all, Britain sided with Germany instead. Second of all, so did Sweden. Third of all, the US joined too late to save Europe. After a treaty was signed in Versailles, Germany became a world superpower. After the war ended in 1918, not many major events happened. Japan invaded China (again) and the USSR invaded Finland. In 1920, however, Japan broke their alliance with Britain, and declared war, because the British had occupied French Indochina. This started the Anglo-Japanese War, which, after Britain called in Germany, turned into the War of the Pacific. The Imperial Japanese Navy (IJN) would clash with the Royal Navy and the Deutsches Ostasiengeschwader, but and invasion on land would not be a thing for the first few months of the war.
Why is france occupied?
The German leadership wanted a friendly france and some colonies not that much of France. It is more likely that German military is stationed there.
Also why did Sweden break its neutrality?
Austria-Hungary would not take that much land as there would be several problem enforcing it on the Italians many of whom hate Austria.
Why would Japan pick a fight with both Britain and Germany?
It would be more prudent for them to focus on China.

No chance Britain would side with Germany, they had been rivals since the 1900s at least and Wilhelm II was very anglophobic.
If the aristocracy had a bit more power in Sweden at the time, Sweden joining the central powers is possible though.
Belgium wouldn’t be fully annexed by Germany, probably just Wallonia.
France wouldn’t lose any of its mainland to Germany, just a majority of its African colonies (except maybe Algeria), France would probably be put under a German client state of some sorts possibly with the House of Orleans
Italy wouldn’t be annexed, perhaps some of their land would be taken by Austria Hungary up in the North but no chance that Italy would be annexed or even puppeted.
Eastern Europe looks fairly realistic
What happened in Spain? So Spanish Guinea and French Madagascar are governments in exile, kind of like Brazil during the Napoleonic Wars for Portugal. Pretty interesting concept.

Japan didn't pick a fight with Germany, Germany attacked Japan for attacking Britain, which they did because Britain was getting too much land in Asia, and Japan wanted to create a Pan-Asian Empire.

Spring of 1921: Japanese forces invade Thailand, and take out the country in a few weeks. With this, they are able to invade British Malaya and the Borders of The British Raj. Japan annexes Indochina, creates the State of Greater Thailand and prepares for an invasion of Sarawak. Seeing the opportunity, protests spring up in India. They would soon turn into rebellions.

Winter of 1921: Hindu and Muslim rebellions have begun in India. Japan has almost complete control over Burma. They also declare war on China(of course, it's Japan).
 
Why has Austria-Hungary annexed the southern-half of Italy and the Ottoman Empire annexed all of Greece?

AH would immediately fall apart from all the new ethnicities it just introduced.

Iran was ruled by the Qajar dynasty, a very incompetent dynasty. The people hated it. Communism was a new ideology that claimed to free the people. Rebellions happened. The Soviets de facto rule the northern parts of Iran, although they are still technically at war with the Qajar's.
