With a POD not before 1800, how to make a scenario where the two main powers today are the UK and China, and they are in a Cold War situation.

In the 1800s China is rule by more reformist emperors and pulls a Meiji. This modernized China becomes enemies with Japan which is allied with Britain. So China joins the Central Powers. World War 1 happens and the US doesn't join but China does so the Central Powers win. In the 1930s a Pacific War happens and China crushes Japan. So a German-Chinese Central Powers alliance face off against an Anglo-American alliance.

Welll...
A) China
Tricky parts. We must have the Qing industrialise eartly, or 'pull a Meiji'. Especially humiliating defeat causes unrest, suddenly Emperors/rebel Emperor/Taiping? decide on course of industrialisation. Taiping would actually work... Though Emperors would too.
Japan is vassalised, as is Korea, Russia is stunted. Indo-China might end uyp Chinese. Then, we have China growing...
B) UK.
Easy. Keep the Empire. However, how to keep it?

B) Anglo.French.Union.
While the US have crumbled in the Civil War and since remained divided into the US proper, the New England Federation, the Free City of New York (the single richest state on Earth an uber-Monaco), the Confederacy with her "Negro bantustans", Louisiana, uber-Texas (who had gobbled up northern Mexio up to the Pacific), California, Cascadia and the Indian territories, a stripe from Oklahoma to Montana where Native Americans fight with white colonists for supremacy.

Taiping? Weren't their quasi-Communist policies doomed to fail?

They weren't really quasi-Communist. They had the basic policy package of any newly industrializing nation, with a fair bit of weird Christianity and a war economy. So, tariffs, land reform, rationing, etc....
