'Fuhrer of the East' if he's still fascist? 'Comrade Fuhrer' for the communist variant?
As for what ideology he would follow, it depends on how he is brought up and the political climate of Russia in the ATL. If everything up to 1917 goes nearly the same as OTL, it doesn't make much sense for a fascist movement to take off unless *Hitler started it, but it isn't that hard to get *Hitler to join the Bolsheviks if he has grown up in that environment.

As for how Hitler could become fascist, how about this:
He grows up in the Russian Empire after being adopted by a white Russian family. Young Hitler grows up embracing Russian nationalism and looks down on the other ethnicities for being backward and needing the Russians to prop them up. World War 1 happens and Hitler decides to join the army. Russia ends up losing territory after the war, Hitler becomes furious at this. Having grown up in the Russian Empire, Hitler wants to create a far stronger empire. Hitler participates in the White Movement and the Whites win, maybe due to Lenin and Trotsky being killed in 1918 or the Bolsheviks infighting among their leadership about who would succeed Lenin. Hitler blames territorial losses on ethnic minorities (including Ukrainians, Kazakhs, and Germans) within the Russian Empire. Hitler like in the OTL Soviet Union, uses the bureaucracy to appoint people he likes in positions of power until he is the leader of the oligarchy of Russia. The 1920s and 30s see massive industrialization and mass deportation of ethnic minorities as aggressive Russification takes hold and minority languages are wiped out. Millions die as ethnic republics are deliberately starved like the Holodomor in Ukraine. Collectivization heavily favors ethnic Russians. Hitler's goals are to expand Russian "living space" west into Poland, the Baltic states, and Germany, as well as to slaughter the Japanese and destroy their empire as revenge for the defeat in the Russo-Japanese War. In particular, Hitler has a deep-rooted hatred for Germany for the conflict them and Russia had in the First World War, and his ultimate goal would be to exterminate the Germans and claim their territory.
Alt-WW2 breaks out when the Russians pursue an invasion of Poland and the Baltic States and Finland to reclaim all lost territories of the Russian Empire.
