Hard to tell since it never happened in history (the mob having politicans in the pocket is one thing, but if they dared to reign in open things become difficult). They'd certainly face one problem: Leading a criminal group is one thing, leading a country is another.

We already have it. The mob is nothing more than a government for criminals. The government nothing more than criminals for government. Pols are pols, it doesn't matter if they rule by force of the gun or force of the vote, they still obtain the concensus of the governed by a combination of bribery and fear backed by ruthless force and a willingness to use it.
The American mobsters of the late 20's all studied and admired Ancient Rome and modeled their organisations on it. So did the Founding Fathers in 1787.
There would be some differences. Mobsters would be unlikely to switch lies as much as the Bush adm has done in Iraq, they realize that you have to know your story and stick to it.

I remember an episode of Red Dwarf where they traveled back in time, and kept Oswald from killing Kennedy, and the mafia took over when Hoover was elected President. Via blackmail, they became the de facto rulers of the US.
