Nice narrative timeline. I look forwards to seeing where this goes, especially considering that the house of Sassan actually lives still - in Tang China.
 
The Assyrian Christians of the Levant may rather have a Persian overlord rather than Roman ones. So if this becomes an "arabs get roflstomped" TL then there is a good possibility of the Levant being Persian.
 
Mesopotamia would likely be Persian, or contested between Persia and whatever remains of the caliphate. The levant is relatively easy to conquer, supply and rule for whomever has the strongest fleet in the Mediterranean, but very difficult to seize and support troops in for the people who have to cross the Zargos mountains and the Jordanian steppe.

OTL, the arabs moved their centre of gravity north of Arabia very quickly, with Cairo and Baghdad becoming the primary demographic centres. They also settled en masse in what they (like the christians) saw as the holy land. Even so, the crusaders were able to wrest much of the levantine coast off of them for a short while, because naval access is so very useful in terms of transport and resupply.

In short, I cannot see Persia holding the Levant for any serious length of time unless the Romans develop a phobia of boats.
