Temporary Survival of Roman Rule

Quite a radical change to history and highly unlikely: For some reason acknowledging the eventual loss of Rome's previous conquests, Roman troops outside of Italy, retreat back to Italy, Corsica, Sardinia, and Sicily, and defend these areas from foreign dominance in the decades and centuries to come (Germanic tribes, Muslim invaders, Holy Roman Empire/Austria, France, and Spain). As Europe changes, this surviving Roman Empire does. The radical deviation from OTL is: Only strong leadership could have ordered the troops to withdraw from Rome's previous conquests and defended them from foreign dominance, meaning, possibly a general launches a coup and his family maintains his lineage as a dynasty. The only conquests made by this surviving empire is that which the Italian Empire did in OTL (Libya, Somalia). Suppose this Roman dynasty of emperors continues until 1939, when Benito Mussolini drags Italy into Hitler's war. How different would Italy be with extended Roman rule? No negative comments please, I understand that this is very radical and unlikely.
