"Super-massive" Black Death

(Before you ask, I couldn't have posted this in the ASB thread because it bars mass death scenarios.)

Let's say, that, around the time of the Black Death kicking off in Europe, a mutation occurs, causing the Black Death to kill at a faster and more efficient rate, while also making corpses and sick people extremely contagious. This causes all of civilization, excluding Sub-Saharan Africa, Oceania, (Southeast Asia up to Malaya could see polities surviving, put not anything more above that.) and the Americas, to largely collapse with death rates averaging out at ~75%, some places seeing much higher and others much lower.

What next?
 
Which kick off of the Black Death? There was more than one, you know. Why do we (and I include myself) automatically think people will know the dates we're thinking of when we post a query?
 
Mutations like that count as evolution and evolution PODs are ASB, so this technically shouldn't be here either, no?

Also, why would plague hit the Americas pre-Columbus? Even with Vikings, the Atlantic Northeast was sparsely populated and far from major population centers of the Americas.
 
No, Africa, Oceania, and the Americas are the only places that survive this plague.

And I guess this situation doesn't belong here either, but if not here, where?
