Obviously OTL we have the West/East Berlin split for an example of a Capitalist exclave, and from time to time the concept of a West/East Vienna crops up as well. But what changes could result in an example of a Communist exclave in the "first world"?

What comes to mind for me is a worse Soviet performance/better Anglo-American performance on the Western Front which means that the Soviets don't get to occupy Germany at all, but that seems fairly unlikely without both sides being much more hostile, and then why would they allow the Soviets any part of Germany? Another option would be a divided Japan following Operation Downfall. Alternatively, a more successful USSR could result in the Allies deciding that a brand new, non-Berlin capital would be best, and we get West/East Frankfurt or something.

Also, what would the exclave look like? The world's biggest and most visible Potemkin village? That's what seems to be the most logical to me.
