Robert Bork put on the Supreme Court

Since Anthony Kennedy, now the "swing vote," was Reagan's second choice, I'd say it would make a big difference today.

The Judicial effects will be interesting (how conservative would Bork be...?), one wonders how this will alter the process of nominating SCOTUS candidates. Bork was, IIRC, the first nominee to face quite such vehement scrutiny. Maybe his successful nomination slows down the polarization of the court and the politicization of the nomination process?

It will be a much more strict constructionist court.

He will probably raise the bar on intellectual arguments both in oral arguments before the court and in the writing of opinions.

He will have an even more important role in the interpretations of anti-trust law.

He does not have an expansive view of the Second Amendment and might not have supported the plaintiffs in Heller vs. District of Columbia.
