Realistic way Stalin dies in the 1930s

What would be a realistic way Uncle Joe could die in the 1930s that is not a total handwave? ( Someone taking him out doesn't count )

Did Stalin have any potentially life threatening health conditions or was he exposed to situations that could pose increased risk of accident or illness?

Did he have any close calls in the 30s?
 
Given the general lack of antibiotics in the 30's it would not be too tough for him to get an infection or disease that kills him.
 
Chokes on his own vomit after too much vodka?

Slips down some stairs?

Just show him a picture of the world after 1991. He'll have a heart attack.

In all seriousness, an aneurysm can be random, spontaneous, and life threatening. He was born with it, and an injury (even a minor one) sets of the aneurysm, and he dies relatively quickly (Like collapsing, and dying before he hits the floor).
 
Stalin was a fairly hardy sort. He didnt have any major health problems in the 1930s. Even through he smoked, was a glutton and drank like a fish...

So you'd need some sort of random illness (or whatever). That could happen to just about anyone and couldnt be treated by the U.S.S.R's best doctors in the 1930s.
 
What about simple mechanical failure. Train wreck or airplane crash might work. Not everything in the USSR, even the equipment used by the top leaders, might not be up to snuff. Or just simple human error. NKVD guards a railroad junction between two track and not understanding how Railroads operate give the local folks orders that end up in a train wreck involving Stalin's train.
