Question about Spanish royal houses

I'm thinking about an alternate timeline where Andalusia revolts at the same time as Portugal. One thing I was confused about is if this guy became King:

Would his royal house be "de Guzman" or "de Medina Sidonia"? I'm probably being idiotic but I'm not very good with royalty. If someone could explain how it all works it would be appreciated!
 
Probably Pérez de Guzman would be the name of the family, since Perez is the paternal name, but then again it is a very common one and not nobiliary at all.
Medina-Sidonia couldn't be, after all it is not his name, but he just the duke of Medina-Sidonia.

If I know anything about the noble families, it's very complicated on what would be the house name, the case in point being both the Dukes of Alba (which despite their last name apparently being "de Toledo" were known as the House of Alba. Although there was a House of FitzJames that was Dukes of Berwick in England and later Spain, there was also the House of FitzJames that were the Duke of FitzJames in France.

The Noble House is in fact called the House of Medina Sidonia, despite the fact that most of them are called Guzman.
