Presidents stay in office indefinitely

I have a feeling someone else would set the precedent. Adams or Jefferson or someone. It probably wouldn't be as powerfull, though, as Washington himself setting it.

Could custom have become that a Presient would serve until they died or lost an election?
Possibly, but a number of presidents desired to leave office but felt compelled to serve a second term. If it was a job for life they may not even have a second term if they found it so undesirable. In which case you may have a few oddballs.

I accept that some OTL Presidents would not even have run in the first place if that was the expectation.

By the way how was Jefferson's physical health?

if he were able to and expected to would he have served til he died or would anyone have beaten him in an election

Would he have been beaten? Probably not. That being said, even if the expectation was to serve until death I don't think Jefferson would go beyond 3 or 4 terms.

This USA would be more like a monarchy except for the name. Not that this would have to be a bad thing.

They are still voted on every four years. Its the expectation that the office should be carried till death or being defeated in the presidential election rather than the two term limit that's been changed.
