How would the direction of road traffic circulation work in this world? even though most countries in the world already adopt the right side as the dominant road traffic side, some countries such as Austria, Spain, Portugal and Sweden still traveled on the left side, while in Italy and Holland the traffic side depended on which region you were in, which led to very strange situations where you had to switch sides of the street while driving constantly.
 
Are the Ultramarine Provinces as important as the Portuguese homeland? For that matter we need a discussion on how Portugal is able to keep Germany and Britain away from the colonies, and not going into debt

It would very likely get standardized similar to OTL. Italy and the Netherlands will pick a side and stick to it.
If the entirety of Austria-Hungary stays on the left side it could influence other parts of Eastern Europe to adopt left-side driving, but it is unlikely.
Sweden is surrounded by right-side driving countries, so it will likely make the switch as in OTL (though even in OTL it was quite late to do so).
Spain and Portugal will only keep left-side driving if both of them keep it. Any change from one means the other has no reason to keep left side except for being a nuisance.

Considering this and being a little biased towards the left side of the traffic we can say that we would have Europe driving predominantly on the right in the north (with the exception of the United Kingdom) while the Balkans, Italy and Iberian peninsula would remain driving on the left, something like this:

Since Germany is closest ally and trade partner of Danubia, I would imaginate Danubia eventually picking right-side driving. Probably Italy too will do same.

Can Italy still fall to an authoritarian "fascist" regime in this world?
If so, how would it work without mussolini? it could be something like Portuguese/Brazilian Integralism or Carlism, or it could be a common military regime.

No, it won't rise like OTL

Fascism Rise in the first place from the Ashes of the Great Wars and people who sacrifice million of their own boy and mens felt betrayed by having little lands for a Massive cost.

Fascism wouldn't even existed in TL without that Betrayed and losses National pride after WW1.
Italy will be like other countries before the War, making treaty's, Expansion of their Empire and allied to the strongest countries at the time

Spanish troops landed on the coast of Morocco during the Spanish-Moroccan War of 2002
The Spanish-Moroccan War of 2002, also known as the Third Moroccan War or War of the Hundred Days, was an armed conflict fought from October 29, 2002 to February 5, 2003 between Spain and Morocco.
The war started after a border incident on 1 October 2002, when a Moroccan soldier was shot by a Spanish sentry, this escalated into an unprecedented diplomatic crisis, relations between Spain and Morocco have always been tense, since their independence from France in the 1980s, Morocco was seeking to retake the Spanish territories in the Sahara, the war of independence in Algeria gave the Moroccan government great hopes.
Fighting between Spanish and Moroccan forces began on October 29, despite the initial Moroccan advance, Spanish forces pushed Moroccan troops back to the border and even bombed the city of Rabat, the war came to an end after France threatened a intervention in morocco, with an armistice being signed on 5 February 2003.
After the end of the conflict, the Status Quo Ante Bellum was declared, which remains until the present day.

You might want to try to find a different picture, because I know that is an Iowa class battleship, and their are pictures of both German and Japanese battleships of the early twentieth century out there.
