Loss Of An Entire Apollo Prime Crew

Come up with a POD that involves the loss of an entire Project Apollo flight crew and a substitution of the backup crew into the mission. Please be specific

It happened to Gemini when a T-38 crashed. The backup crew took over, as planned. Killing an entire Apollo crew wouldn't be too hard (unfrotunately): helicopter crash, airplane crash (albeit T-38 were two seaters, and Apollo was three man).

I would say a collision between two T-38 trainers in poor visibility or because of a technical glitch. NASA actually lost four Apollo-era astronauts in three T-38 crashes between 1966 and 1970.

Astronauts used T-38s not only for training but also for ultra-fast travel across the USA and the myriad of contractors and NASA centers scattered all over the country.

Say, your Apollo crew is flying to a contractor or NASA center aboard two T-38s and then they collide in poor weather, killing everybody aboard.

there backup crew Walter "Wally" Schirra Jr., Donn F. Eisele and R. Walter Cunningham flew in Apollo 7 (AS-205)

in this case we have Apollo 7 and 13

in Apollo 7, Schirra developed a severe head cold during mission and infected his crew mates in capsule
hat that happen during quarantine before launch they had be grounded and Backup crew take over

in Apollo 13 CM pilot Ken Mattingly, was exposure to German measles. so the flight surgeon grounded him and John Swigert took his place.
The prime crew could get grounded in fear they all could get sick in space (see Apollo 7) and Back up crew take over

A T-38 crash wouldn't stop Apollo, just like it didn't stopped Gemini, since a T-38 crash in not related to Apollo (unlike Apollo 1).
 
Was there a procedure for crew T-38 flights? I know in the Gemini 9 disaster, See and Bassett were in one T-38 and Stafford and Cernan were in a second one
