I recall hearing of such state referred to as "Zacoslavia", or some variant, like "Zachoslavia" or "Zakoslavia".

Outsiders may call them simply "Slavia".

Seconding this.

IIRC the names for the various slavic monarchies in Bohemia, Poland, Ruthenia weren't used, or even haven't had been set up before the XIIIth century. Until then, the lands in the east were collectivly referred to as "Slavdom" with no distinguishments (apart from distinguishing the realms of the various ruling noble houses). So a pan-west-slavic state is likely to become "Slavia", "Slavica", "Regnum Slavicum" or something like this.

I'd vote for "Regnum Slavicum" if that was a option.

Are you looking for their name for themselves or how the outsiders would call them?

What the outsiders would call them.

Or Slovakia, a name for an OTL country of the Western Slavs.

A Latin form could be Sclavenia or Sclavinia. (the same basis of OTL Slavonia and Slovenia). I suppose that Germans would call it "Wendland" or something similar.
If the PD is far back enough, "Moravia" is a possibility.
In general I suppose that the dominant tribe would easily give the name to the whole thing, like happened to Poland OTL.

Depends on the POD...

Late Antiquity/Early Medieval: Possbily "Lechia"
Early Medieval: Probably "Moravia"/"Great Moravia" ("Venedia" or "Wendland" to outsiders)
Medieval: (Harder to determine... Possibly something Latin like *"Slavica", "Venedia", or something along the lines of "Sarmatia")
19th/20th Century: "Zapadoslavia" (don't forget the "o" in there... "ZapadSlavia" sounds as awkward as "YugSlavia")

*"Sclavonia/Sclavinia" was traditionally used to refer to South Slavs.

Or Slovakia, a name for an OTL country of the Western Slavs.

Possibly, if such a state was formed early enough and led to the polish, czech and slovak languages becoming even more similar to each other than OTL and the west slavs would not come to self-define themselves by territorial occupation. If the name was to derive from the original slavic word for "slav" (slověn) then "Slovakia" or "Slovenia" are a possibilty for an "anglicized" name.

However I find it more likely that the english term for such a state would derive from the german and/or latin term for it, like OTL - ger. Bohmen - Bohemia (not Czechia) ger. Polen - Poland (not Lechia). So ger. Slawen - Slavia.
