As I'm far too lazy to create a timeline of my own for this scenario, how do you think this could be achieved? Or atleast the Repubbliche Marinare staying independent. :)

Considering how different the speech of North and South Italy is, there could easily be 2 or 3 states that unify 'their' languages.

I split Italy into two pieces in my TL. Although that is more over ideology then over ethnic differences..

No, I'm interested how all the Pre-Unification Countries/City States (Republic of Pisa, Venice, Florence and so on,) can survive to the present day.

Naples could get all of the parts of Italy south of La-Spezia-Rimini if it retains Lazio while the Duchy of Milan could get all of the northern parts of it
 
I imagine that without Garibaldi you could easily end up with Sardinia-Piedmont in control of the north with a surviving Kingdom of the Two Sicilies in the south and the Papal State between the two.

Just having the Papal State survive could have all kinds of interesting effects...

You'd need a very early POD. Your best bet is essentially trying to tweak events so that nationalism never becomes a powerful force. How you do that...I'm not sure, it's not easy. Perhaps have France manage to keep small parts of North Italy, maybe England keeps some land on the continent, the Aragonese Empire stays strong, etc, so that you have a world where multinational empires are commonplace, and having nation-states seems less of a good idea since it means all the big states having to make unpopular gestures of handing back land, which they will never do. Of course, this will likely mean that Italy remains a frequent warzone...
