I suppose some particularly gifted or charismatic individual could pull a Napoleon during the 19th Century struggles for independence from Spain, achieve dominance over the pro-independence forces, drive the Spanish out without American assistance, and set himself up as a monarch. The rise of an extraordinary individual who was never born IOTL is always an easy POD.
 
How would the US react though? If it is a European, similar to Maximilian in Mexico, the Monroe Doctrine might be invoked, antebellum South forces the US to get involved, after the Civil War the US has less reason to but more capacity with all that equipment, seasoned veterans, and manufactoring capacity. If this is around 1830s or 1840s prior to Mexican Amercian War (1848) then Mexico has a legitimate legal claim to Cuba and may put forth their right to a protectorate and this could lead to US Mexican strain and war earlier, an earlier war MAY favor the Mexicans.
 
Wouldn't it just be easy for the Spanish monarch to give Cuba, the Philippines and Puerto Rico a similar status Britain gave to Canada, Australia and New Zealand. The Spanish King would be declared Emperor and the colonies are his overseas realms. Nah?
 
Do a variant of the Aranda Plan, making the various colonies Dominions, and putting Spanish (or related) royals on their thrones. That would be the easiest way.
 
We don't have that very special individual, but we have some talented people who could help. For example, General Prim, while his service in Cuba, gathered very good relations with some cuban separatist leaders. As the clever and liberal man he was he understood not only the rightness of their demands, but also that an independent Cuba was only a matter of time. Fate wanted to make him the strongman of Spain after the Glorious Revolution of 1868 that toppled Isabel II, and it was his pet project to bring a liberal and democratic monarch to Spain, leading to the proclamation of Amadeo of Savoy as king of Spain. However, fate also wanted to get him murdered some days before the arrival of Amadeo, murdered in uncleared circumstances. Writing from a phone us hell, sorry. I'll finish my post here. I said Prim would probably pushed for a dominion status for Cuba, under a constitutional monarchy. He had the charisma, popularity, intelligence and the balls to go ahead with this in Madrid, plus the right contacts in Cuba.

This seems like a very cool POD to give the Carlist movement far more strength, as well as a Cuban monarchy (well, Carlos would no doubt still claim to be the real King of Spain) which isn't the same as the Spanish monarchy by a Canada-like law (probably the most likely).
 
There is a Darkest hour mod in which there is no american spanish war, so in 1936 the Spanish empire "federalizes", with Cuba, the Phillipines and Morroco becaming "independent" but each one with a member of the spanish royal family in the throne
 
Did any of the Spanish Royalty ever visit their colonies? Being the Spare could give Don Carlos the opportunity to do such a thing, and visit one of Spain's remaining possessions. How would the population of Cuba receive the arrival of a Prince. It could be the starting point for Carlos if he is forced to flee.

I think that it never happened before, but in 1833, before the First Carlist War, Carlos was exiled in Portugal. He could have been exiled to Cuba instead... but we need a more energetic Carlos.
