How likely would it be that John Kerry actually picks John McCain in 2004 to be his running mate?

I think Kerry would have been delighted to have McCain as his running mate. The question was whether McCain would accept, and I consider that extremely unlikely. By 2004, McCain's eyes were already on the grand prize in 2008.

The only possible way I see anything remotely like this happening is for Bush to have another VP, perhaps Lamar Alexander or Robert Smith, or even Pat Buchanon in 2000 and again in 2004. Perhaps Cheney gets to be White House Chief of Staff.
If its someone else who is likely to get the grandprize in 2008, then McCain might give it some stronger thought.

Considering the fact that Kerry actually did ask McCain to be his VP, the answer to your question is "extremely".

The question to ask is what would have been needed for McCain to accept.

Have him get snubbed like Jeffords was snubbed. Have Bush, whom McCain was still very bitter towards, organize Republican events and specifically exclude McCain. Shouldn't be very hard, Bush didn't feel any need to appease McCain till after 2004.

So, have McCain go turncoat. The Dems were courting him for it IOTL. Then, come 2004, he is a Republican-turned-Democrat and might see Kerry's offer as the only way to get a say in any future administration.
 
I guess it depends on how the term goes. If Kerry is running again, McCain, obviously, is out of the question. Assuming that the major players remain somewhat similar, I think Mike Huckabee is the most suited to attack a Kerry-McCain ticket's weaknesses.
