Islamic Extremism in a continued Cold War

What would happen if both the USSR survived and Islamic fundamentalism became seen as a source of global insecurity? Would OBL still be seen as the greatest enemy or would he be downplayed to keep us focused on the "evil empire"?

Given that the Soviet Union would be inherently under threat from revolutionary Islamist sentiment (and this was already present in the USSR thanks to radicalized segments of the Tajik and Uzbek populace from the Afghan War), I'd say it might be one of the few things that could get the two sides to cooperate.

I'll go along with that. Revenge on the Soviets for the Afghan invasion, and dislike of Americans for the 1990-91 DESERT SHIELD/STORM period, gives those people two big enemies. And a common enemy for the two superpowers. Watch for the Soviets to turn Afghanistan into a live-fire area for their latest ordnance, and occasional Spetsnatz/Alpha Group raids going after the more...obnoxious individuals (and UBL would certainly qualify).
