Okay, so Senator Stephen Douglas dies of typhoid before January 1853. The Kansas-Nebraska Act as it was never comes to be, though someone would probably come up with an alt-version in a misguided attempt to ease North-South tension at a later date.

Pierre Soule also dies soon after October 1854 in Madrid and news of the Ostend Manifesto is never leaked.

Do the Pierce or Buchanan administrations try to pull it off (considering Buchanan helped author the thing)?

Does Spain acquiesce and sell Cuba or risk war? What course of action does the US pick if they decide to put their money where their mouth is?

What are the repercussions on North-South relations and the alt-Civil War? What are the repercussions for future US expansion?

Spain probably isn't going to sell, and the US will be too internally divided over the issue to actually risk war.
