Help Remembering a book title

I've been trying to find a book that I read when i was younger, but i can't remember the title. Its based in New York during world War 1, and an American teenager makes friends with a Russian immigrant, who eventually dies of either pneumonia or spanish flu, I can't remember. Also: the kids sister has a baby that he helps deliver, they talk about how much they hate the czar


Finally, I have a feeling the title is something like : Happy Birthday, ....... !
