Truman was picked because the party bosses knew FDR was going to die and Truman was acceptable to be President. He clearly took something of a shine to Truman, hence his selection as VP. So Truman may all have been the nominee. I suppose that he might have given the nod to William O. Douglas, but it's hard to say.
Granted, a world where FDR survived would be wildly different.
I doubt it. He was in very poor health by the start of the 4th term. If he lives out the fourth term, and doesn't resign after the War to go to the UN, as many not just on this site have suggested, he would step aside after four terms and give someone else a try.Would it be possible that FDR would run fifth term?

Why Douglas? I understand he was a close contender for VP in 44' but did FDR like him that much more than Truman?I suppose that he might have given the nod to William O. Douglas, but it's hard to say.
How so? Surely his policies wouldn't be wildly different from Truman's?Granted, a world where FDR survived would be wildly different.
