How would this affect the Cold War without Austria and most of Germany along with a small party of Italy now communist
 
You likely get a stronger US postwar settlement in Asia.

With a larger block of territory, you see a Prussian Socialist Republic and an Austrian Democratic Republic, with a small piece of Germany going to the WAllies as West Germany. Finland, at best, gets a harsher version of Finlandization. In compensation, Stalin "pays" a symbolic sum to the WAllies for the additional territory, and a not-so-symbolic price at the diplomatic table, particularly with Asia.

US pressure with the Japanese surrender leads to Stalin stabbing Mao in the back as "unstable" and a "traitor to the workers' revolution" - leading to the Nationalists winning the Civil War, thanks to the Soviets. His condition is that the Soviet Union get an Independent and Finlandized Manchuria (possibly with or without Inner Mongolia) with a 99-year Soviet lease of Port Arthur and the right to base naval and military assets for its Pacific HQ there.

Would it actually make any difference?

- Once Berlin and the east starts to collapse the west will fall and the US can simply drive at high speed east to meet not far to the west of OTL?

- Had the official post war occupation divisions not been pre agreed so that the forces will simply move to the OTL agreed line after a bit of stealing by Soviet troops?

Not possible. Not with only a few weeks of delay. All that means is that 3rd Army crosses the Rhine in early April.

There is no way the Red Army can realign in that time. Stalin had put everything into taking Berlin, not believing that Churchill and FDR (not to mention Ike) literally didn't have any desire to pay the butcher's bill for the place, and all of his forces and logistical support was set-up to support the capture of the City.

In any case the only way the WAllies can be hurt this badly is if the Heer has an additional 15 full strength motorized divisions and the logistical train to support them. If the Heer had that sort of strength the Red Army would still be in Poland in April of 1945.
