So what would the consequences be if one of Alexander's generals managed to unite his king's empire. And maintain it until contact with the Roman's.

Allegedly on his death bed when asked by his generals who his successor would be he replied "the strongest"

None of them were OTL but WI one of them was?
 
Nothing would change significantly.

In any case, the closest one to reunifying Alexander's empire was Seleucus. Just have him not be assassinated by Ptolemy Keraunos and Seleucus will succeed in conquering Macedonia and Greece, leaving the entire eastern Greek world save Egypt under his hands. It likely wouldn't last long after his death.

I don't know, but Alexander naming his successor is out of his character.
But let's assume it happened for whatever reasons.

Yes, that general would have more chances to keep the Empire united. That would probably mean changing the dynasty - Alexander's kids and other relatives would "accidentally" die.

And the pattern of the united world empire might stick for some time, following Achaemenid standards. So the periods of unity might be followed by the periods of disintegration.

Actually the former Alexander/Achaemenid Empire was close to (almost) uniting several times even in OTL.
 
If the empire of Alexander the Great and could exist only in two ways - either need an adult heir (though also not deprived of talent), a revival of one of the descendants of the Diadochi. Even if Alexander failed to appoint a receiver (or Regent), they still started, would gnaw each other's throats.

The story is potentially apocryphal-but according to certain historians he did say something to the affect of "The Strongest"-in Greek of course.
