How possible would it be for the title of Roman Emperor to die in the West after the title fell into disuse before Otto was crowned by the Pope?

It's been more than a century after Charlemagne at this point. The POD is the gap between Berengar I and the crowning of Otto I.

No, not possible. That genie is out of the bottle. Eventually someone in the west will manage to repeat what Charlemagne did and will demand and demand the Imperial title, which the Pope would probably give. The only way this wouldn't happen is if the Byzantines were somehow about to reconquer Italy. Then the Eastern Empire would have a presence in the west again, have control of the Pope and be able to deny any Western "barbarian" the Roman title.
 
@ Emperor Constantine: besides those "effeminate" 'Greeks' were way too busy in the East, to really do something against those "barbarian" 'Franks'.
In fact that's one of the reasons, another one was religious, that the Papacy turned to the strongest proper Roman Catholic power for protection (instead of the Byzantine Empire).
