Cuban Missile Crisis POD: Gitmo gets Nuked

so I was reading Then Everything Changed the end of the first AH story has LBJ in charge of the Cuban Missile Crisis, a Russian on the ground decided to attack the US Navy Base at Guantanamo Bay with Nuclear arms, what would happen if A) in OTL that had happened and B) the US lunched a full Nuclear Attack on the USSR (thinking Gitmo was the first blow in a full Soviet attack), no one in Moscow knows of a Cuba bomb, would the USSR be able to mount any kind of counter-attack? how badly would 1962 USSR be hit? how badly would the USA be hit?

Possibly US could achieve total surprise and get most if not all Soviet missiles on the ground, without fuel. By the time they are fueled US Minutemen are already falling on known launch sites. Soviets attempt counterattack with bombers, some brake through, others fail.

Some Soviet missiles from Cuba and remaining ICBMs from USSR hit major cities in USA. DC, New York, Chicago and few others. Major damage there, but nothing compared with devastation that has befallen USSR.
