Beyond climate concerns (three field systems require rain more than two field systems) and soil concerns (extra plowing is helped by heavier plows)...

What are the factors homding up someone from saying 'hey, being able to harvest 4 of 6 fields instead of 3 of 6 is a great idea,' and introducing three field crop rotation.
 
Basically better, sturdier plows, as far as I can reckon. Thats the big factor in making three-field rotation work.
 
There is still a dispute amongst historians and archeologists, if the romans already used (at least partially / regionally) three field rotation or just the two field rotation. If they actually did, this is another lost roman technology.
Unfortunately the archeologic evidence for such agricultural processes is hard to get, but there are some hints.
