Overall I plan for him and Natalya having 6 kids overall (marriage in 1672) in time window 1672-1681, before the disease starts progressing too much for funny happy bedtime (Ivan V's OTL issue being as example), Michael II being a firstborn (and having 2 to 3 surviving siblings, that is 3-4 surviving kids out of 6).
