War Plan Orange by Matrix. A strategic/operational simulation of a hypothetical naval war between the US (and UK) and Japan in the early 1930's. Several scenario variants depending on whether or no Washington Treaty signed and all or some of the cancelled ships were completed. Very detailed.

IL2 Sturmovik 1946 by IC/Ubisoft. Last version of the classic combat flight simulation, includes a "what if" campaign and flyable aircraft based on an Alternate History in which WW2 continues for several more years, including among other things a brief German civil war between the Wehrmacht and Nazis/SS. A surprisngly well-developed AH backstory to what is basically just an opportunity to fly around in German experimental protorypes and early Russian jets.

Crimson Skies, by Microsoft. Another flight sim-based AH game. Collapse of US during great depression, lots of new countries, sky pirates, zeppelins, wierd '30's planes, good story and voice acting. Pretty arcady, but what's not to like?
 
Wasteland is another "alternate history" game since World War III occurred between the US and USSR in 1998 and you start off in 2087 exploring the U.S. Southwest. You get a group of PC's together and you can even recuit NPC's as well. It was made in 1987 for the Apple ][ series, PC, Commodore 64 and some others too.
