Assuming a victory at Osaka Castle, I could see it, although Hideyori would be a bit silly to allow one of his retainers to attain a rank only recently abolished at the time and a clear challenge to his authority. The Imperial Court had to invent ranks to give to Hideyoshi in the first place, due to his low origins, and the title of Shogun and Kampaku being in two different people would create dissent and division, however loyal Nobushige was to Hideyori.
If you kill off Hideyori shortly after Ieyasu it can be done, perhaps a wound just as the Tokugawa fall back in one of the mooted plans for him to lead a charge against the Tokugawa personally? Nobushige takes charge, with the help of Goto Mototsugu, takes down Hidetada and the rest of the Tokugawa, and then declares himself as Shogun whilst granting the rank of Kampaku to Kunimatsu as Hideyori's son. Have Kunimatsu die before he reaches maturity, or prove to be a bit of a bastard and be stripped of his rank, and you have your Sanada Shogunate.
