With a PoD after its creation, how could the United Nations have failed? Failure can include anything from a permanent UNSC member withdrawing all the way to the United Nations de jure or de facto ceasing to exist.

Maybe the USSR walks out for good during the Berlin Airlift crisis? I'm thinking it would have to be fairly early, before inertia and convenience make it not worth the trouble.

Are there even any provisions in UN law for evicting a member? (As opposed to recognizing a different government of the same member nation, as with ROC/PRC)

Nuclear War at any point would end the UN. It will have by that point in time failed.

I think if the Soviets get pissy about the Missile Crisis not going their way more so than OTL, there might be a chance.
 
If the UN fails, a new UN/LoN/you-name-it would be created after the mess where the UN failed at.
 
Am not sure that the "UN" is not a failure. Certain parts of it like the WHO are working, but most of the UN agencies are dysfunctional or minimally functional at best. The bureaucracy is loaded with no show or patronage jobs dealt out to all members, and usually used for corrupt purposes to appoint relatives etc. for well paid jobs. UN "peacekeeping" is, usually, a joke - UN rules of engagement really don't allow any intervention just wag a finger.

Compared to the hopes/plans for the UN...

I agree with what other posts have said. If a nuclear war were to occur, the UN has failed at its primary goal.

How about the Soviets never returning after their boycott which started in January 1950 protesting not giving the UN seat to the PRC after their victory over the RoC in the Chinese Civil war? They stayed away for months IOTL. Maybe this would result in an Alt-UN for Communist and friendly countries plus the original UN for Western Allied and friendly countries.
 
The United Nations essentially is a failure. It's a toothless entity which in large part never really found an answer to the problem of national sovereignty and competing interests of various powers.
