AHC: The Queen refuses to shake someone's hand

She's shaken the hand of Nicolae Ceausescu, easily the worst of the Warsaw Pact dictators after Stalin and, IMO, the most evil European leader since Stalin. Once you've done that, someone would have to do something really bad in order to get snubbed. Without massive changes to OTL I wouldn't say it's possible.
 
She's the Head of State, doing the meet and greet with dubious/unpleasant/murderous people is part of the job. If she really objected then a face-saving excuse would be created.
Yes, that's really the point of this thread - the exercise is finding people who would require such an excuse, with bonus points for an unavoidable obvious snub (incredibly rare from Her Maj).
She's head of Plaid Cymru now so I imagine they've met and shaken hands loads.
Those two statements don't necessarily follow - Plaid aren't in the Welsh Government so in the grand scheme of things as far as HMQ is concerned, Leanne is Just A Woman. But they appear to have shaken hands once, though Wood did indeed refuse to curtsey.
 
The Queen is very old school British. She believes in duty over everything.

If she has to meet someone she'll do her duty and treat them as she'd treat anyone else she meets. If there was someone she had a deep personal reason not to meet (in the example of the IRA killing DofE or one of her children) then I imagine it would be easy enough to engineer an illness but considering she met McGuinness even after the murder of Mountbatten (a great favourite of the Royal family) I think it's possible she might even put duty over self then.
