No, this isn’t a trolling, lets make an interesting exercise, you can do the PoD in the Middle Age or whenever you want, the conditions are:

Poland owns parts of Germany like Bavaria, the Rhineland or Saxony.

The polish king is related in some way to Charlemagne, his descendants, other German noble families or Swedish ones

Poland is still catholic.

Lithuania is part of Poland.

At least one king of Poland is named Adolf.

Well, what do you mean, then? A Polish kingdom that doesn't unite with Lithuania? Poland conquering Germany? Give us a little more to work with here, please!

This isn't too hard, I think. A union of Saxony with Poland-Lithuania under the Wettins is plausible, particularly if the Great Northern War goes differently. Thereafter, a King Adolf of Poland can be easily accomplished by just having one of the rulers like the name.
 
We already have someone like Augustus III of Poland of the House of Wettin. He was King of Poland and the Elector of Saxony within the HRE.
 
The Habsburg dynasty has some hard times starting 1500 or so and the Jagiellons replace them as the dominant dynasty in that part of Europe.

a duke of Saxony-Poland is named adolf. Seperate administrations but I think it's the best shot you have
 
Goes without saying that, for anything like this to be possible: a) ditch the elective monarchy (or never adopt it in the 1st place) and b) curb the power of the Szlachta...

Absolute latest possibility - complete Napoleon-wank... Prussia-screw at Tilsit:

When the Treaty was being formulated, it was noted by an observer that the Prussian king was pacing on the bank of the Neman river; Napoleon had to "but raise his hand, and Prussia would cease to exist" (McKay).

Duchy of Warsaw (later elevated to Grand Duchy/Kingdom of Poland, dependent upon the whims of L'Empereur) receives Silesia (uniting Saxony and Poland), West and East Prussia...

Followed by success in Russia (borderline ASB already I know...) Saxony/Poland expanded eastward to the 1793, possibly even the 1772 boundaries...
Eventually "The Empire" collapses under its own weight, but Saxony-Poland remains...

What happens after that is basically anybody's guess :) How "Polish" this state would actually be is also a good question...

Alternately, and even later, you could try to wank Pilsudski's Promethean scheme... but that's even trickier... ;)
