Now before beginning this,I want to say that this is about a more united European peoples and not a completely united European pouplace.
So what is a POD when European peoples are comparitively more united? Drag the POD to as behind as you want. So a different Indo European migration, more domination of Europe by Caucsian and Anatolian origin Indo-Europeans or Caucasian peoples like Hurrians and Urartians,Greek domination over all of Europe with Alexander,Persian Conquest(Alexander nd Persians were more benevolent which would not anger the native tribes and instead, probably, they could come into the Empire) are some scenarios I can think of.

I mean in the Middle Ages.
Communication issues, and the general Feudal structure during the error make it hard to establish effective control over large areas. the Kingdom of France was spending half of it's time putting down unruly vassels and what not. For a medieval 'united europe' It would have to be a direct contiuation of rome, with the buy in to the imperial system from the provinces or the people would just decide F- it and break off.

Mainly for mediaeval era but you can come up to Napolean or someone. Also stick to Native European and Christian empires only.

In the Middle Ages.

I'd just go with cutting down the number of 'small' countries by causing a general failure of separatist, decentralization, and independence movements.
1. Portugal never achieves independence from Castile, and Castile unites with Aragon OTL. United Iberia after the Reconquista.
2. Scotland never re-achieves its independence during the reign of Edward II. England can then focus that attention to subjugating Ireland. Great Britain.
3. Margaret the Great's son, Olav, never dies and so she we don't have Erik becoming king. Olav turns out a better ruler in the vein of Margaret. United Scandinavia.
4. The Duchy of Burgundy is fully subjugated by France early, allowing France to all but absorb its Low Country territories.
5. The Hasburgs have better luck against the Ottomans, not enough to push them out of Europe but enough to prevent Hungary from partially falling. The Ottomans then turn towards Arabic pursuits, and the Hasburgs can focus on centralizing the Holy Roman Empire. The Low Countries not part of France stay in the HRE.
6. Poland-Lithuania manages to avoid the Golden Liberty era, partially preventing itself from its later decline. It stays a major power in that region.
7. Gian Visconti doesn't die when he does, and subjugates northern Italy into a new proto-Lombard Kingdom. Eventually the Visconti are subjugated to the Hasburg Emperors, making northern ?Italy part of the HRE. Naples, Sicily, and Corsica are absorbed by Aragon to eventually be part of Iberian Union.
8. Russia unites all of Europe east of the PLC, early pushing out the Crimean Khanate and going to the Caucasus.

There. While its numerous personal unions, but you've got basically all of Europe split between a smaller number of major regional powers. It isn't exactly a Napoleonic superstate, but this way there's like ten major monarchs dominating 95% of Europe.
