Here are some quick thoughts:

2003: Actually both sides would ask him, to what extent will he defend "Roe v. Wade", would he support the Supreme Court overturning Roe v. Wade? Does he support federal funds being used for Planned Parenthood?

2004: Calling someone a "gay Chinaman" is considered highly offensive by the Asian-Pacific Islander community. To many it is tantamount to calling someone "chink". To do this in Palo Alto, California would invite immediate protest...

2006: Hezbollah and Al-Fatah would use the withdrawal to attack Israeli troops, making TR look like an idiot in front of the Israelis...

2006: Supporting a bill, however well intentioned, that deploys military troops to the American borders will:
A) Cause Mexico to take offense, and call for withdrawal from NAFTA, which would anger the business community, and many Republican donors
B) Insult the Latino/Chicano community. Consider that the only person who has proposed this in OTL has been the Minuteman Militia, the Tea Party, and Jan Brewer (R-AZ). This has caused the community to support the Democratic Party...

Based on these comments alone, this looks like a "Macaca" disaster...

what does that actually mean? what sort of thing would a person have to have done / be in order to be thought of as a Great American? Any examples?

Well, obviously it's subjective, but for a start you could take a look at the non-presidents on this list, or the lists of similar countries -- also think of Americans who are generally celebrated by and/or loved by the nation (anyone hailed as a hero would be an example...)
