Well, the basic foundations would almost certainly be Russia vs. Austria.
I wonder if you could have something come out of the Treaty of San Stefano and the Bulgaria established in that. Russia pushes for the treaty to be enforced, Austria allies with the Ottomans and I would think Greece to try and reduce Bulgaria's power, while Serbia, Romania and Montenegro are something of a toss up. I would lean towards being on the Russian side due to fearing Austrian or Ottoman domination more than a Bulgaria which an alliance between the three of them, Greece and if necessary the Ottomans could take down (Not to mention Serb claims to Bosnia-Herzegovina). Italy would want to dive in and nab Albania, Germany would probably support Austria, while France though not officially allied with Russia may decide that this is a good moment to try and pounce on a distracted Germany. Britain probably remains neutral.
