Ah, they way you framed your statement was that it was ordinary people doing it, not police. And it wouldn't just be Afrikaners doing it, there would be English-speakers too and perhaps even black police.

Mandela dies and the situation completely spins out of control. The white minority radicalizes and unleashes a wave of massive violence. Control splinters and black rebels gain control over some parts of the country. In these parts we have a Communist South African Republic.

The agitation of the communist party has to be more successfull, untill they have enough support for a revolution.
But there has to be the right historical situation for this to happen. Maybe Mandela is killed in prison, labour strikes are broken up, government shoots into the crowd, the ANC is seen as too moderate (with Mandela style peacefull resistance having obviously failed), the communists are seen as the alternative, the USSR is seen as a better partner tha the US (that has backed the Apartheid regime for so long), etc.

Hm. If the USSR survives, South Africa could easily end up in a civil war... And depending on how that goes, you could have a communist faction win, or a leftist faction wins that becomes communist due to that being the side really willing to help them.
I suspect it would be other leftist African regimes that would intervene most in a SA civil war, but you could have the USSR and its satellites being pulled in more directly in the civil war or post civil war period.
