AHC: Basileus elected HRE king of the Romans.

Is this feasible at all?

Perhaps if the Eastern Roman Empire and the crusaders cooperate well and a charismatic Basileus aids a crusader army containing a couple of electors? Possibly either when Henry IV or V dies during one of their low points or when Frederick I dies on crusade.

Could the Byzantine Empire hold onto Germania and Italy?
 
There was a possibility of the reverse of that happening during Otto III's reign. Just before he died, Otto III was bethrothed to Zoe Porphyrogenita, daughter of byzantine emperor Constantine VIII. Had Otto not died, he would have married her, and possibly produced a son, who could end up as both Holy Roman Emperor and Byzantine Emperor. I, however, doubt such a personal union would last, and there's the possible obstacle of Otto having two sons, demanding each realm for each one.

But it might recreate the polite fiction of one Roman Empire, which for a few generations could lead to some actual cooperation. Surely not as effective as the old Roman Dominate, but still.
 
The bloated, bureaucratic, incompetent, decentralised Nightmare of Voltaire crossed with the country with politics so volatile and complicated that we still use it's name to describe absurdly underhanded or complicated political situations. What could possibly go wrong?

Ok, that's a bit harsh. But I don't see it working well for either of them. Depending on the time period, the dominant partner likely does some legal black magic to steal the subordinate partner's Italian lands, at least partially. I expect a military coup in the East or a failure to secure the next election in the HRE within a century; and the other side's nobility will take this as an opportunity to break away. Somehow this will inevitably end with a crucial Byzantine institution destroyed and the HRE reverting any attempt at centralisation.

If it did survive, it would be glorious. There would be little dispute over a stable union's claim to Rome, especially if Rome is subsumed into the Empire.

Even half a century of Glorious United Roman Empire(s) would count as a success in my book. Moreso if it does not end in catastrophe and thus leaves the door open for something like it happening again.
 
Byzantines discover gunpowder in the 10th century. By the 13th they have a proto-gunpowder empire that reconquers the East. Then the Mongols come, and this time Ogedei doesn't die prematurely. They conquer Hungary and Poland while carving into Mesopotamia, right on the Imperial border. A massive German army led by the HRE tries to stop them, but they are destroyed and he dies. In a desperate effort as the Mongols near Bohemia, the Electors choose the Basileus, who had been making noise about reuniting Rome, as Emperor to try to stop the Mongols.
