How can you get a single dynasty, family, or clan to possess at least three of the following titles?
- Khagan
- Latin Pope
- Greek Patriarch
- Western/Holy Roman Emperor
- Eastern Roman Emperor
- Caliph

Well, the Ottomans sort of did. They were Khagans, Qaysars (Eastern Emperors) and Caliphs. And they pretty much controlled the Greek Patriarchy, though not having the title themselves of course.
 
Well prior to the Great Schism and the evolution of the Papacy Rome was counted as one of the 5 Patriarchates along with Constantinople, Alexandria, Antioch, and Jerusalem. Prevent the Schism or butterfly some details and you can have the Pope continue to use the title Patriarch. Western Emperors is also pretty easy, the Papacy was frequently held be scions of various noble dynasties especially as the Middles Ages wore on and some of the German noble houses were very big, with lots of cadet branches. Eastern Emperor is more difficult, though if the Latin Emperor counts then that shouldn't be too hard either, it passed through a number of Western noble families before being crushed so if we put it into the hands of our Germanic family then we're looking good.
But the first and the last look impossible Caliph is an explicitly Muslim title and I can't see how that can be combined with Pope while Khagan seems equally impossible.
 
OK well the Constantinian dynasty come to mind. I think that one of Constantine's sons ruled the west and the other the east. Maybe instead of the third son getting a section of territory to rule, he is instead made Pope/Patriarch? Or the Valentinian and Theodosian dynasties. Have a third or fourth son go into the clergy and be made Pope/Patriarch.
 
But the first and the last look impossible Caliph is an explicitly Muslim title and I can't see how that can be combined with Pope while Khagan seems equally impossible.
It just means successor in Arabic. Alternate, more successful crusades might even see the leader of the crusaders calling himself the Caliph of Asia or the Christian Caliph. Since Crusaders did really love Arabic culture and language.

Or, the Umayyad Caliphs in al-Andalus could have converted to Christianity.
 
It means Successor to Muhammed, which means its intrinsically linked to Islam.
Not all forms of the word have that meaning. Plus the Crusaders could misinterpret it.

And there's always the Caliph converting to Christianity option (pretty likely in Iberia).
 
Option 1:

More successful Byzantine reconquest of Italy. One of Justinian's successors decides to lock up control of the church by appointing his younger sons at Patriarch of Constantinople and Pope, while holding Emperor of ERE for himself and his eldest son.

Option 2:

Henry IV (HRE) fares much better in the Investiture Controversy, gaining control of the Papal selection process. His successors continue to be heavily involved in crusading, and as per OTL a crusade eventually winds up redirected to Constantinople. The HRE acquires the *Latin Empire and merges the Imperial titles, and also controls appointment of both Pope and Patriarch of Constantinople.
