AHC: A Baltic Union

Unite the Scandinavian states, the Baltic states, Prussia and Poland-Lithuania under either annexation, personal union or federation. Bonus if they can prevent Russia from becoming a great power until the mid-18th century.

This is the best I can do--a more successful Sweden in dynastic union with Prussia which seizes West Prussia and Lithuania from Poland before dominating that whole country politically with the installation of a friendly candidate then beating Russia badly on the battlefield. A continuing Triple Alliance of Sweden/Prussia, Poland and the Ottoman Empire should keep Russia hemmed in indefinitely. It would be bad news for Austria as well.

Sigsimund III never gets deposed from the Swedish throne. Or alternatively, the Swedish Deluge succeeds or he or his heirs reconquer Sweden. The trick is to make a union stick, which requires smart, skilled kings capable of finessing the formidable obstacles of religion and powerful nobles without getting overthrown in either country.

Sweden and Poland-Lithuania were briefly united OTL. If such a union stuck you're most of the way to fulfilling that challenge, and the rest can plausibly be accomplished through conquest. Brandenburg-Prussia, Denmark-Norway, and Russia would probably try to break it up, as they opposed Sweden OTL, but Poland-Lithuania is a large country and if it was securely unified with Sweden's excellent troops I could picture Sweden-Poland winning said war decisively.

Get a personal union between Sweden and Prussia some time in the late 1700s.
